  • Asking your provider about any documentation you may need to bring.
  • Collecting all current medication bottles to your appointment, including prescriptions, supplements and over-the-counter medicines, to bring to your appointment.

Bill Payment

Many, but not all, CHMG providers are part of our centralized billing system. If you received a statement from Patientco, visit the Cone Health patient financial services page to learn about our billing and payment experience.

If your doctor’s office does not use the Patientco billing system, contact the office directly with any questions. You’ll find the practice’s phone number on your bill.

Electronic Medical Records

Most Cone Health Medical Group providers use Cone HealthLink, an electronic medical records system that integrates information from different locations and systems together in one computerized medical chart. Learn more about the advantages of Cone HealthLink and who has access to your chart.

Electronic Request

Log into your MyChart account where you can print and send information to health care providers directly. If you don’t have a MyChart account, request account activation here.

Paper Requests

If you’re not on MyChart, or the information you need is not available on MyChart, download and complete the Cone Health Medical Group Release of Information Form [PDF]. (The form also is available in Spanish.)

Release Form Instructions

  • Get the practice name, address, phone number and fax number needed for the first box on the form. You’ll use the practice name again in blank after the phrase “I authorize Cone Health or”.
  • Print or electronically fill in the form. Complete all applicable fields. You must include the purpose of your request.
  • Be sure to select which information you need disclosed. Most health care facilities do not require or want your entire record.
  • Sign and date the form, and then submit it to your doctor’s office. (Electronic signatures aren’t accepted.)

Please note: If you want your copies sent to another health care facility, first ask what it requires and whether it’s on the EPIC health information management system. If it’s on EPIC, you may not need to complete the request form.

EFFECTIVE BEGINNING JUNE 10, 2021 - MEDICAL RECORD FEES FOR PATIENTS:

Charges may apply based on how you want your records delivered. Paper records may only be available in paper delivery. Prepayment is requested prior to releasing medical records. An invoice is sent directly to the patient/guardian for patient directed requests. You will not be charged when requesting us to send medical records to another healthcare provider.

If you (the patient/guardian) want records sent to anyone other than yourself or a healthcare provider: 

  • You may be charged the NC State Rates for records you are requesting to go to a third party; (rates listed below).  (Examples of third parties: attorney, life insurance, someone other than the patient/guardian).

North Carolina State (records copy fee) information, per article 29 – § 90-411.:

NC State Rate

For the first 25 pages 1-25 pages .75¢ per page
For pages 25 through 100 26-100 pages .50¢ per page
For pages exceeding 100 101+ pages .25¢ per page

If a third-party requests records on a patient:

  • The third-party requestor may be charged the NC State Rates; (listed above)(Examples of third parties: attorney, life insurance, someone other than the patient/guardian).
